If you have a concern or complaint about your care received at CGH Medical Center, please contact our:
Patient Advocate at 815.625.0400, ext. 4011 or in writing to: CGH Medical Center, c/o Patient Advocate, 100 E. LeFevre Road, Sterling, IL 61081

If you are not satisfied with the handling of your concerns, ask our Patient Advocate to schedule a review by our Grievance Committee.

CGH Medical Center is a Joint Commission accredited organization. If you have a patient safety event or complaint that has not been answered by CGH Staff or Administration you may contact: The Joint Commission:
