Our Request an Appointment is not a confirmed appointment, and does not guarantee an appointment with the physician or provider requested. This service is not for emergent conditions. A member of our staff will confirm the details of your appointment and contact you during normal business hours: Monday through Friday from 8 a.m. to 5 p.m. We will make every effort to assist you in finding a provider if you do not have one. Please wait to receive your appointment confirmation before coming to the hospital or clinic.
All requests are confidential and are reviewed on a case by case basis. Please note, due to current provider shortages in family and internal medicine, it is possible that we may not be able to place you right at this time.
