After 15 years, the faith-based clinic, Agape Care Team Health Services, located at 16 E. Miller Road, Sterling, will be closing its doors on November 1. Co-founder and Director, Connie Cato, PA-C, has been preparing patients for the clinic’s closure since May of 2010.
“This has been a valuable service to our community for many years,” said Ed Andersen, CGH Medical Center President and CEO. “Connie Cato and her staff are to be commended for their service to those in need of health care services.”
Since May of 2010, the primary service of Agape Care Team Health Services has been helping low income and uninsured patients obtain medications, diabetic supplies, limited lab testing and x-rays. Each patient was required to have a primary physician and was seen at the clinic at least once a year.
Agape patients have been trained on how to obtain their medications through their primary care physician. New clients are being referred to a list of local nonprofits.
Agape Care Team Health Services opened in August of 1997. It started in the basement of the Rock River Christian Center by Hasmukh Shah, MD (CGH Tampico Health Center), Jyotsna Shah, Connie Cato and her late husband, Harrel. Their mission was to give needed healthcare to the low income and uninsured in Whiteside County and surrounding areas. In 2001, they moved to the Whiteside County Health Department until October of 2006. Since that time, Agape has been at two locations on E. Miller Road, first at 14 E. Miller and at present, 16 E. Miller Road.
Agape Care Team was run by a team of caring healthcare professionals who volunteer their time and services at no cost. The only paid positions were a few part-time office support staff and part-time nurses. Office Support includes: Shirley Robertson, Debbie Bowman, Mary Carboni, Cassie Tornow, Alison Phillips, Zach Rosene, Adrian Lopez, Duncan Robertson, Maria Lopez, Dan Rivera, Tricia Gaumer and Deanna Coers. Healthcare Providers include: Hasmukh Shah, MD; Dr. Dahiya; Jan Andersen, NP; Melissa Taylor, PA-C; Connie Cato, PA-C.
