1. To assure prompt delivery, please enter the patient’s full name
2. Greetings received after 4 p.m. may be delivered the following day.
3. Greetings sent on weekends or holidays may be delivered the following business day.

Note: Greetings received after a patient has been discharged cannot be forwarded to the patient’s home address.
