Cardiac Wellness Project Programs designed to educate people in our community on ways to prevent the
#1 cause of death - heart disease. About 1 in 3 Americans has heart disease. The
CGH Cardiac Wellness Program aims to reduce the incidence of cardiovascular disease
by identifying and educating individuals who are at high risk for heart disease,
stroke, arteriosclerosis, diabetes and hypertension. Health educators conduct
monthly community cholesterol, blood pressure and glucose screenings and host
a major event in February during Heart Month.
Colorectal Cancer Prevention Program Promotes screening of individuals age 50 and older of this highly preventable

Diabetes Prevention Education focused on identification of pre-diabetes and prevention of type 2
diabetes. Did you know that 1 in 3 children born today will develop type 2 diabetes?
The rates of childhood diabetes and obesity have tripled in the past 30 years,
leading health experts to predict this current
generation of children will have a shorter lifespan than their parents. In addition,
51 million Americans have pre-diabetes, with many children now also being diagnosed
with this normally adult-only disease. This program aims to help individuals identify
whether they are at risk of developing diabetes through education and by holding
blood sugar screenings monthly at various locations. Click here to learn more.
Eyeglasses for Kids Program Free eyeglasses for financially eligible school-age children in our five-county
service area. Almost 2,100 children have received a free eye exam and eyeglasses
since the program started in 1995. Area school nurses are our link to finding

Health & Fitness Initiative (includes the CGH Learning Garden Project) An educational effort to encourage children and adults to alter their lifestyles
through increased physical activity and making healthy food choices.
“Today’s children face serious health problems”, says Dr. Eric Kuhns, family practitioner at CGH Family Practice. “Quite literally, the unhealthy shape of over 30% of today’s young people may hasten the onset of type 2 diabetes, heart disease and cancer well before what’s considered normal.” To counteract the spiraling childhood obesity problem, action is needed. But while children are a major focus, our programs are offered year-round for adults as well.
Changing lifestyles one child, one family, one school, one community at a time is the goal of the Health & Fitness Initiative. The CGH Centennial Learning Garden Project is part of this program. In addition to the gardens, education-based programming is carried out in the classroom, after school, at CGH, at the library, Women Out Walking, and with other partners like Selmi’s Greenhouses, University of Illinois Extension-Whiteside County/Master Gardeners, Whiteside County Health Department and YMCA.
Health & Fitness Initiative offerings include: Centennial Learning Garden Project, Women Out Walking, Cooking demonstrations, Strong Women, Strong Bones class, “Fresh Fridays” in the schools, Health education booths in the workplace and schools, Intergenerational Cooking Club and the CATCH Program at area schools.

Lung Cancer Prevention Program A continued effort to help individuals quit smoking and raise awareness about
the dangers of second-hand smoke.
Men's Health Dedicated to educating about risk factors of prostate and testicular cancer
Prescott Dental Clinic Low-cost dental care for children and adults. Joint effort with the Whiteside
County Health Department. The clinic provides services for children and adults
who meet income guidelines. The Foundation provides support for daily supplies.
Nursing and Healthcare Scholarships Nursing and healthcare scholarships for 25 local youth/adults attending college. Click here to learn more.
Workplace Wellness Program On-site workplace screenings and education centered on lowering blood pressure,
cholesterol and glucose. Community Services staff work with local businesses to
offer low-cost screenings and education to help employees manage their health.
Educational materials, newsletters, monthly screenings,health risk assessments
and walking programs are all offered for a custom plan for each business.Click

Women's Cancer Prevention Program Year-round breast/gynecologic cancer education and services, including financial
assistance for mammograms, Pap tests and physician visits. Includes a major breast
cancer awareness event in October, programs on breast and gynecologic cancers,
Two-for-One Mammogram promotion; Blooming Box health education service; voucher
program for under-served with major partners Team Underwires, Susan G. Komen Foundation
and Whiteside County Health Department.
