Recognizing Your Gift

PARTNERSHIP OF HEALTH
All new members receive a Partnership of Health pin. Donors contributing $100 or more are eligible for a plaque. Contributions are recognized annually in the Foundation newsletter and at the winter reception.

Gift levels include:

  • Friend: $1-$99
  • Member: $100-$499
  • Partner: $500-$999
  • Tree of Life: $1000 or more.

TREE OF LIFE
We invite you to join those whose names are engraved upon the Tree of Life sculpture in our main lobby.

The following donor levels have been established:

  • Leaf: $1,000 to $9,999
  • Apple: $10,000 to $24,999
  • Stone: $25,000 and over.

Contributors with a cumulative giving level of $1,000 or more are recognized during a special Tree of Life dedication each year.

LEGACY CIRCLE
The Legacy Circle recognition program has been created to honor planned gift philanthropists. If you have provided for CGH in your estate plans, please let us know. We would be pleased to welcome you as a member of the Legacy Circle.
